Heads on the east slope of Cobb Mountain, trends east-southeast 11.2 km (7 mi) to Middletown where it continues north 5.6 km (3.5 mi), then east 16 km (10 mi) to The Cedars where it flows southeast for 36.8 km (23 mi) through Lake Berryessa to continue east-northeast 42.4 km (26.5 mi) to end at the golf course south of the community of El Marcero.
